Tailored Support

 No two people are the same, which is why every support plan at Learn to Live is personalised to reflect each participant's goals, preferences, and aspirations. We work closely with participants, families, and support networks to deliver high-quality, person-centred services that make a genuine difference. 

 

What We Offer

At Learn to Live, we provide personalised disability support that empowers individuals to live independently, build meaningful connections, and achieve their goals. Every service is tailored to meet the unique needs, preferences, and aspirations of the people we support.


In-Home Support

Our in-home support services are designed to help participants maintain their independence while feeling comfortable and confident in their own home.

Our team can assist with:

  • Personal care and daily living activities
  • Meal preparation and nutrition
  • Household tasks and cleaning
  • Medication prompts and support
  • Shopping and errands
  • Building independent living skills
  • Overnight and respite support

We work alongside participants to promote choice, independence, and confidence in everyday life.


Community Access

Being part of the community is about more than simply getting out of the house—it's about building relationships, developing new skills, and enjoying life.

Our Community Access supports may include:

  • Social outings and recreational activities
  • Attending appointments
  • Shopping and errands
  • Sporting and fitness activities
  • Volunteering and employment support
  • Learning public transport skills
  • Building confidence and social connections

Every outing is centred around the participant's goals, interests, and independence.


Day Program

Our Day Program offers a welcoming, inclusive environment where participants can learn new skills, make friends, and enjoy engaging activities in a safe and supportive setting.

Activities may include:

  • Arts and crafts
  • Cooking and life skills
  • Community excursions
  • Health and wellbeing activities
